In case of a positive pregnancy test the number of foetuses and their placement in the uterus are screened by ultrasound (US). The US is performed two weeks after a pregnancy test, i.e. at the beginning of the 7th week of pregnancy. At that time, the size of the embryo exceeds 3 mm and the heart action must be apparent. Upon such findings we may talk about clinical pregnancy and evaluate the IVF treatment result as positive.
Pregnancy timing
In natural pregnancies, the day of conception is usually not known, because ovulation may be irregular, within a scope of a week. Obstetricians therefore calculate the length of the pregnancy as 40 weeks /+-14 days/ from the first day of the last menstruation. In pregnancies induced via IVF treatment, the day of embryo transfer to the uterus is known exactly. According to the “obstetric mathematics”, the 3rd week of pregnancy starts as of the day of transfer. Heretofore, a positive test represents the 5th week and the ultrasound examination the 7th week of the pregnancy, respectively.
The term of such pregnancy may be calculated by adding 38 weeks to the date of ET, or two weeks can be subtracted from the date of ET and then the usual 40 weeks added. However, such labour term estimation is just mathematics. The biological maturity of pregnancy and the beginning of the labour are usually strictly individual, therefore full-term pregnancy is considered within the period from 38th to 42nd week.
Treatment audit
If both the pregnancy test and the blood screen turn out negative, it is suitable to perform a treatment audit. The entire therapeutic procedure is analyzed together with both partners. The stimulation cycle is never a mere treatment. It is always also a continuation of diagnostics, since only upon termination of the embryo transfer cycle answers become apparent to such questions as how the ovaries react to stimulation, how the eggs react to sperm contact, how the embryos of the couple develop, how successful the embryo transfer was, etc.
The goal of the audit is to analyze said information and outline new treatment possibilities, taking into account all of the knowledge acquired. Thus, it is possible to further plan a more custom-tailored treatment. Particular conditions of the female organism and of the couple can be taken into account. The positive effect of such refined treatment planning is confirmed by the fact that women become pregnant more easily than during the initial cycle. The audit is also designed to explain to the patients that an adverse outcome of one treatment cycle cannot be considered a final result. It is designed to return optimism and patience to both partners.
Factors influencing conception
The cause of an ever-growing number of couples having difficulty to conceive is largely unknown. The specialists in reproduction medicine mostly hypothesize that this adverse trend is implicated by the increasingly stressful lifestyle and the deteriorating environment. It can be hardly expected that one will change these factors within a few years. However, other adverse factors, such as smoking and obesity, can be influenced by each individual.
Smoking and obesity are considered the most significant factors complicating or preventing conception.
